关于control sheet 和summary sheet.

在summary sheet里的提示是
2009 Mathematical Contest in Modeling (MCM) Summary Sheet 

(Attach a copy of this page to each copy of your solution paper.) 
Type a summary of your results on this page. Do not include
the name of your school, advisor, or team members on this page.

就是说，把摘要打印在下边，并且把本页打印3份，分别放在3份论文打印稿的前面，对么？

在Control sheet里面有提示：
Do not make changes by hand to the information on this control sheet. If you need to change any of the information on this sheet, login via the Advisor Login link on the MCM web site at http://www.comap.com/undergraduate/contests/mcm, make the changes online, and print a new sheet.


Each team member must sign the statement below:
(Failure to obtain signatures from each team member will result in disqualification of the entire team.) 

Each of us hereby testifies that our team abided by all of the contest's rules and did not consult with anyone who was not on this team in developing the enclosed solution paper.

这是说，先打印一份control sheet，然后每个成员要在下面用手写邮件地址和签名，最后钉在3份复印件的最前面，对么？

呵呵，想搞清细节问题。。。毕竟第一次参加美模。。不想犯低级错误。先谢谢各位的回答。
